39 Observed tongs stored next to old plumbing materials on a shelf. Ensure tongs utensils are stored in locations where they cannot become contaminated. 228.124(a)(1)(B) Except as specified in paragraph (4) of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored where they are not exposed to splash, dust, or other contamination.
47 Ensure hand washing signage stating "All employees must wash hands before returning to work" is posted at the hand sinks in the restrooms. Ensure hand washing signage is also posted at the hand sink in the room with the 3 compartment sink. 228.175(e) Handwashing signage.
43 Ensure the light shield is replaced in the room with the 3 compartment sink. 228.174(a)(1) Light bulbs shall be shielded, coated, or otherwise shatter-resistant.
45 Observed peeling paint on the wall, and a small hole on the ceiling in the room with the 3 compartment sink. Ensure the wall and ceiling are repaired. Also observed peeling paint on the second level. Ensure the facilities are maintained in good repair. 228.186(a) Repairing. The physical facilities shall be maintained in good repair.
39 Observed the ice scoop stored on top of the ice machine. Ensure the ice scoop is stored in a clean and protected location. 228.68(b)(5) In-use utensils, between-use storage. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in a clean, protected location if the utensils, such as ice scoops, are used only with a food that is not time/temperature controlled for safety.
39 Observed accumulation inside the ice machine by the office. Also observed accumulation on a margarita dispenser and nacho cheese dispenser in the lobby bar. Ensure the ice machine and dispensers are cleaned regularly. 228.114(a)(5)(D) Surfaces of utensils and equipment contacting food that is not time/temperature control for safety shall be cleaned: in equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (i) at a frequency specified by the manufacturer; or (ii) absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.
39 Observed an ice machine with a broken lid by the office. Ensure the ice machine is repaired. 228.111(a) Good repair and proper adjustment..
39 Observed a box of popcorn containers stored on the floor by the office. Ensure single service items are stored at least 6 inches above the floor. 228.124(a)(1)(C) Except as specified in paragraph (4) of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored at least 15 cm (6 inches) above the floor.

31 The water faucets in the second level restrooms must be held for the water to be on. Ensure the self closing water faucets stay on for at least 15 seconds. * 228.146(b)(3) A self-closing, slow-closing, or metering faucet shall provide a flow of water for at least 15 seconds without the need to reactivate the faucet.
18 Observed chemicals stored next to single service cups at the small bar by the stage. Ensure chemicals are stored in locations where they cannot cause contamination. * 228.203 Poisonous/toxic materials or chemicals stored properly.

NOTE: The City of San Antonio Metropolitan Health District presents food establishment "Inspection Scores" calculated from a perfect 100 score minus any identified violations and subsequent demerits.
