18 Bleach solution, in waitress station, was high in concentration. Ensure that solution is maintained between 50-100ppm.
10 Ware washing procedure was not being conducted properly. Sanitization step was being skipped. Ensure to wash in the following order: 1. WASH (with soap and water) 2. RINSE (clean water) 3. SANITIZE (solution maintained between 50-100ppm)

10 Dishes are not being washed properly. First sink had bleach water, second sink had bleach and soap. Ensure to wash all wares in the following manner: 1. WASH (soap and water) 2. RINSE (clean water) 3. SANITIZE (bleach water at 50-100ppm). Observed residue along interior of ice machine. Ensure to clean and sanitize surfaces of ice machine interior to remove debris.
18 Bleach solution was high in concentration. Ensure that concentration is maintained between 50-100ppm.
35 Observed employees eating and drinking on back preparation area. Ensure that personal items and eating/drinking are done in a designated area away from food and food contact surfaces.
36 Discontinue use of cloth towels being hung from employees' waists. Ensure that all cloth towels used for multipurposes are stored in sanitizing solution immediately after each use.

1 Cheese enchiladas that were rolled and stored in containers, yesterday, were at 71.2 degrees F. Employee stated they were cooled but no temperature or time control was conducted to cool and store foods at 41 degrees F and below. Once food is removed from heat and is 135 degrees F, food is allotted 2 hours maximum to cool from 135 to 70 degrees F. Once food is cooled to 70 degrees F, food is allotted 4 hours more if needed, to cool to 41 degrees F and below.
6 Bacon pieces were at 111 degrees F, off hot holding unit. Ensure that such foods held under time control are logged to show their time frame of 4 hours. If foods remain after the 4 hours, foods should be marked to be discarded, when at room temperature. This is when foods are not being held under proper cold and hot holding temperatures.
14 Observed employee touch face then proceed to write up labels to place on food containers. Observed employee don a new pair of gloves after tasks changed. No hand washing occured at each of these incidents. Ensure that hands are properly washed each time a task is changed.
35 At beginning of inspection, observed employee eating while at preparation table. Ensure that all eating is conducted in a designated area away from food and food contact surfaces.
37 Install a splash guard on both sides of hand sink. Hand sink is next to open bins of flour and ware washing sink. Ensure splash guards are made of material that is smooth, easily cleanable, and non absorbent.
42 Metal racks where microwaves are stored, and other items used for food service, should be cleaned thoroughly to remove grease and dust buildup.
47 Only involuntary grade placard was posted. Ensure that most recent routine inspection report is posted in public view at all times. Grace card can be posted if desired, but should not replace the report.

18 Bleach solution being used for cloth towels in back kitchen and waitress station, were too high. Maintain solution between 50-100ppm. Use test strips. Observed tube of hand sanitizer, sitting on shelf, above single service items. Ensure that all toxics are stored in a designated area away from food and food contact surfaces. Observed pico de gallo and diced corn chips being stored in plastic containers that are not food grade. Observed tray of masa balls rolled, beign stored in non food grade plastic. Food grade bags are present in establishment. Ensure that foods are only stored in food grade storage containers, etc.
32 Gaskets/rubber seals, in refrigerator doors, should be replaced when torn, cracking, etc to prevent accumulation of food debris. Shelves over hot holding area, have peeling paint and should be smooth, easily cleanable, and non absorbent.
37 Observed buckets of diced potatoes in water, sitting on floor. Observed container of flour uncovered, next to hand sink. Ensure that all foods are stored 6 inches or more off floor, and that if needed, a barrier is placed around hand sink to prevent splashing and contamination of foods stored around sink.
45 Observed personal beverages and phones sitting along food preparation areas. Beverages were being removed at beginning of inspection. Ensure that a designated area is provided to employees for their personal use. This area should be away from food and food contact surfaces. Ensure that all personal drinks are stored in a sturdy cup with lid and straw. no bottled drinks are allowed to be used.

Observed food debris build up in gasket folds of cold hold unit that is located next to hot hold. 228.113(3) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. 228.114(c) Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
