29 Observed that the testing strips were expired (had an expiration date of 09/2022). Ensure that the three-compartment sink is always stocked with testing strips so that the staff can check if the sanitizer concentrations are within acceptable parameters (50 to 100 Parts Per Million (PPM) for Bleach/ Chlorine sanitizers, and 200 to 400 PPM for Quaternary Ammonium sanitizers). The testing strips MUST NOT be expired. The CFM ordered more testing strips online. Ensure the following: 4-302.14 Sanitizing solutions, testing devices.
28 Observed a container of seasoned raw pork with stickers that stated “Tuesday”. The inspector educated the Certified Food Manager (CFM) and the Food Handlers that the Time/ Temperature Control for Safety (TCS) food and beverages NEED a label that states the name of the food, the date it was prepared, and the Use By date. Ensure that day of the week sticker labels are NO LONGER USED. Ensure that any Time/ Temperature Control for Safety (TCS) food or beverages, such as Alfredo sauce and seasoned/ marinated raw meats, stored within the establishment are given a preparation date and a Use-By date. The Use-By date has up to twenty-four (24) hours to be labeled on the container from the time it is placed inside a cold hold unit. The TCS food item or beverage MUST be discarded after seven days at the end of the shift (7 days: day of preparation and six days after that). Ensure that the dates are correct. Ensure the following: 3-501.17(A) Date marking prepare on site RTE/ TCS food.
10 Observed that the ware washing machine (dish washing machine) was NOT dispensing Chlorine sanitizer at the end during the Sanitize cycle (testing strips were reading 10 Parts Per Million (PPM)). The Food Handler primed the ware washing machine and ran it again, but the sanitizer was still coming out too low at 10 PPM. Ensure that all dirty dishes and utensils go through a Wash (soap and water), Rinse (clear, clean water), and Sanitize (water with Chlorine/ Bleach) cycle before being placed on the clean air-drying racks. If the food-contact surfaces of these dishes are NOT sanitized/ disinfected, then the NEXT food that is placed on the plate shall be contaminated with leftover microscopic bacteria. The staff continued washing and rinsing the dishes in the ware washing machine, but then they moved the dishes to the Sanitize compartment of the three-compartment sink to submerge them in Bleach water. Ensure the following: 4-601.11(A) Food-contact surfaces clean to sight and touch. 4-703.11(C)(1) After being cleaned food-contact surfaces shall be sanitized an exposure time of at least 10 seconds for a CHLORINE SOLUTION. 4-501.114(A) Chlorine sanitizing solution shall have a minimum temperature based on the concentration and pH of the solution as listed in the following chart.
22 Observed that one chef was still missing their Food Handler certification. Ensure that all staff members who are open-handling food and beverages (cooking, baking, preparing, modifying) become Food Handler certified. The inspector highly recommends that the establishment holds a copy of each Food Handlers certification in a binder on site to present to the inspectors during each inspection. The establishment and Food Handlers are responsible for renewing the certifications before they expire. New employees have thirty (30) days from their hire date to become Food Handler certified. Ensure the following: 228.31(d) Food Handler Training criteria.
42 Observed that the roof/ ceiling surface inside of the ice machine had black and dark grey substances growing on it. The inspector educated the Food Handlers that they NEED to use a towel that is soaked with Chlorine sanitizer (50 to 100 PPM) to wipe off the debris that forms inside. Ensure that all Nonfood Contact Surfaces are cleaned and sanitized with Bleach/ Chlorine solutions that have a concentration of 50 to 100 Parts Per Million (PPM), or with Quaternary Ammonium solutions with concentrations of 200 to 400 PPM. This shall prevent the growth of pathogenic microorganisms that can contaminate food. Ensure the following: 4-602.13 Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

30 Observed that the new Food Permit for the October 2023 – September 2024 fiscal year was NOT posted. The inspector shall issue a 10-day Notice of Violation (NOV) for the owner to purchase the food permit for this year. Inspector was informed by Fiscal that a late fee shall be included. Ensure that the establishment owner/ representative purchases the new Food Permit before or during the first month of the fiscal year (October). 22 Observed that two chefs did NOT have Food Handler certifications. 39 Observed a handle of a flour scooper buried under the flour. Ensure that all food scoopers/ utensils are stored in the food with the handle above (the handles are grabbed by hands, so the handle touching a food item will contaminate it). Scoopers may also be stored in their separate bucket/ pan if the bucket/ pan is cleaned and sanitized frequently. Ensure the following: 3-304.12(A) In-use utensils, between-use storage. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored except as specified under subsection (B) of this section, in the food with their handles above the top of the food and the container.

42 Observed that the top slide-open cooler that stores wines had food debris along the top edges, and the bottom of the cooler had food debris accumulation. Ensure that the inside surfaces are frequently cleaned and sanitized. Ensure that the cardboard placed inside is changed out frequently to prevent bacteria growth that could contaminate the beverages. Observed the inside ceiling/ roof surface of the ice machine have black, grey, and white substance accumulation.
