Observed the ice scoop, and paddle were stored on top of the ice machine. These locations are not kept clean at a frequency needed for the storage of those items. Store in a location that is kept clean.

2 * 228.75(f)(1)(B) Cold Hold (41°F or below). Temperature Control for food Safety (TCS) foods must be maintained at or below 41 degrees Fahrenheit. Observed food items in the walk in refrigerator were at around 44 degrees Fahrenheit. The ambient measured temperature was 45 degrees. The integral thermometer for the walk-in was at 44 degrees and fluctuating to 42. The documentation for the morning check of equipment had the unit at 39 degrees Fahrenheit. While assessing the unit, the integral thermometer kept rising till it measured 48 degrees indicating the cooler was just now failing. Recommend icing down the food to bring it quickly to 41 degrees Fahrenheit. The unit must be serviced to bring it back into compliance.

NOTE: The City of San Antonio Metropolitan Health District presents food establishment "Inspection Scores" calculated from a perfect 100 score minus any identified violations and subsequent demerits. The demerits system is computed based upon the number of violations found during a regular food establishment inspection.
