31 Observed that the handwash sink behind the front counter was missing paper towels, it had a fork stored inside of it, and it did not have hot, running water that reached 100 degrees Fahrenheit (F). Ensure that all kitchen and bathroom handwash sinks are stocked with hand soap and disposable paper towels during all hours of operation. The hand soap CANNOT be a bar of soap or liquid dish soap. It MUST be liquid or foam hand soap. Ensure that the basins of the handwash sinks are kept clear (should only be used to wash hands). Ensure that all kitchen and bathroom handwash sinks have cold and hot running water. The hot water MUST reach a MINIMUM of 100 degrees Fahrenheit (F) during all hours of operation. The employee on site placed paper towels closer to the handwash sink and shall speak to the owner whether to return the hot water or remove the handwash sink completely. The establishment has another handwash sink to the right of the ice machine that reaches 105 F. Ensure the following: 5-202.12(A) Hand washing facility provide water at a temperature of at least 38 degrees Celsius (100 degrees Fahrenheit) through a mixing valve or combination faucet. 5-205.11 Using a hand washing facility. accessible at all times for employee use; not be used for purposes other than hand washing. 6-301.11 Hand washing cleanser, availability. 6-301.12 Hand drying provision.
42 Observed that the ice machine had a little amount of black substance growing on the inner ceiling/ roof and on the front white surface. Ensure that all Non-Food Contact Surfaces are cleaned and sanitized with Bleach/ Chlorine solutions that have a concentration of 50 to 100 Parts Per Million (PPM), or with Quaternary Ammonium solutions with concentrations of 200 to 400 PPM. This shall prevent the growth of pathogenic microorganisms that can contaminate food, and prevent insects and rodents being attracted. Ensure the following: 4-601.11(C) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. 4-602.13 Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
24 Observed that the bags of ice were missing labels that state: The name of the food, the business that made the bags of ice, and the address of the business where the bags of ice were produced. Packaged foods and beverages REQUIRE a label with the following information: 1. Common Name of the food item or beverage. 2. Name of the business/ establishment 3. Address of the kitchen where the food item or beverage was prepared/ cooked. The establishment CANNOT sell the bags of ice until the labels have been placed on the bag. The establishment did have labels for the bags of ice, they just did not place them on the bags. The employee on site began placing the labels around the drawstring of the bag. Ensure the following: 3-602.11(A) Food packaged in a establishment, shall be labeled as specified in law. 3-602.11(B)(4) Label include the name and place of business of the manufacturer, packer, or distributor. 3-602.11(B)(1) Label include the common name of the food, an adequately descriptive identity statement.
35 Observed a personal paper food bag stored on top of a cardboard box of beer inside the walk-in cooler. Ensure that all employee beverages have a secure lid/ cover and stored below Ready To Eat (RTE) and Time/ Temperature Control for Safety (TCS) food items on a tray (tray catches liquids if the drink falls over). Employee beverages may also be stored in a drink holder rack that is added to a wall. Ensure personal food items are stored underneath RTE food items or beverages that are sold to clients. Personal beverages and food should NOT pose as a contamination risk of customer foods. Ensure the following: 2-401.11 Eating food, chewing gum, drinking beverages, or using tobacco.
45 Observed broken glass on the floor inside the walk-in cooler. Ensure that all walls, floors, and ceilings are frequently cleaned and sanitized to prevent accumulation of food debris (which can attract pests). Observed holes in the wall behind the door that leads to the three-compartment sink and a hole in the wall underneath the three-compartment (bottom right corner where plumbing leads to). Ensure that all walls, floors, and ceilings are smooth, easily cleanable, non-absorbent, durable, and in good repair. Observed miscellaneous equipment in the back storage room where beers and sodas are stored. Ensure that the back storage room is cleaned and cleared of any item that has no purpose within the establishment. Allowing equipment and items to clutter within the back room will create ideal harborage conditions for pests such as rodents and cockroaches. Ensure the following: 6-501.12 Cleaning, frequency and RESTRICTIONs. 6-501.11 Repairing. The physical facilities shall be maintained in good repair. 6-501.114 Maintaining premises, unnecessary items and litter.
29 Observed no testing strips at the three-compartment sink. The testing strips are NEEDED when the staff Wash, Rinse, and Sanitize food-contact utensils or dishes such as the ice scooper and any tongs used to handle pickles. Ensure that the three-compartment sink is always stocked with testing strips so that the staff can check if the sanitizer concentrations are within acceptable parameters (50 to 100 Parts Per Million (PPM) for Bleach/ Chlorine sanitizers, and 200 to 400 PPM for Quaternary Ammonium sanitizers). The testing strips MUST NOT be expired. Ensure the following: 4-302.14 Sanitizing solutions, testing devices.
