2 Inspector took temperatures of beans (in three separate spots) in the walk-in cooler and got the following temperatures: 50 degrees Fahrenheit (F), 50 F, and 50 F. The beans were made on 10/22/2023. The inspector discarded the beans. Ensure that all Time/ Temperature Control for Safety (TCS) food items are kept at 41 F or below within walk-in coolers, reach-in coolers, cold holds, and refrigerators. If TCS food is out of temperature (above 41 F) for more than four (4) hours, then it MUST immediately be discarded. Ensure the following: 3-501.16(A)(2) Cold Hold (41°F/45°F or below).
18 Observed a white spray bottle on the windowsill above the three-compartment sink that contained degreaser chemical inside but it was NOT labeled. Ensure that all spray bottles filled with chemicals or pesticides are labeled on the outside with the name of the chemical. Staff may use a dry-erase marker or a sticker label to indicate which chemical is inside. Ensure that the label is rewritten/ replaced once it becomes too faded or scratched out (unreadable). Ensure the following: 7-102.11 Poisonous/toxic materials or chemicals not in original container labeled. 7-201.11 Poisonous/toxic materials or chemicals stored properly.
42 Ensure that the outside surfaces of the pina colada and margarita machines are frequently cleaned and sanitized. Ensure that the sliding edges and the floor of the sliding coolers are frequently cleaned and sanitized. Observed that the inside walls and inner ceiling/ roof of the ice machine had an accumulation of black substance. Observed dirty inside surfaces of the container where the catfish are stored inside the walk-in freezer. Ensure that all Non-Food Contact Surfaces are cleaned and sanitized with Bleach/ Chlorine solutions that have a concentration of 50 to 100 Parts Per Million (PPM), or with Quaternary Ammonium solutions with concentrations of 200 to 400 PPM. This shall prevent the growth of pathogenic microorganisms that can contaminate food. Ensure the following: 4-601.11(C) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
45 Observed a hole in the bottom part of the wall behind the water heater. Observed a hole in the ceiling above the mop/ service sink. Observed a hole in the floor to the bottom-left of the handwash sink. Observed a hole in the ceiling to the upper-right of the window. The metal sheet flooring to the entrance of the walk-in freezer was loose and warped. Ensure that all walls, floors, and ceilings are smooth, easily cleanable, non-absorbent, durable, and in good repair. This shall reduce the number of points of entry for pests. Ensure the following: 6-501.11 Repairing. The physical facilities shall be maintained in good repair.
34 Observed dead pests behind the grease trap and below storage racks. Ensure that all dead pests are swept up and thrown away. Ensure the following: 6-501.112 Removing dead or trapped birds, insects, rodents, and other pests.
34 Observed a gap above the mesh screen for the window. Observed daylight peering through the bottom of the sliding mesh screen exit door. All entrances and exits, including windows, MUST be tight-fitting. This shall reduce the number of points of entry for insects and rodents. Ensure the following: 6-202.15(A) Outer openings, protected.
37 Observed frost and ice making direct contact with smelts and large tilapia fish within the walk-in freezer. The fish were discarded due to environmental contamination. Ensure that frozen food does NOT make direct contact with ice, frost, or water in order to prevent environmental contamination. Ensure the following: 3-303.12(B) Unpackaged food may not be stored in direct contact with undrained ice.
