10 Findings: Gallon containers stored in reach-in cooler. According to employee, they reuse the containers for slushie mix and company that services the slushie comes once a month for replacement. Containers are only washed but no sanitizing occurs. There is also no dish washing detergent located at 3 compartment sink, only a bar of body soap but there is a bottle of bleach available for sanitizer.
Corrective Actions: Inspector gave and reviewed the 3 sink dishwashing method handout with employee. Ensure that all equipment and utensils that are foo-contact surfaces are washed, rinsed, AND sanitized.
4-703.11(C)(2) After being cleaned, food-contact surfaces shall be sanitized in a contact time of at least 7 seconds for a CHLORINE SOLUTION of 50 mg/L.
18 Findings: Can of stripping paint, Fabuluso, and other bottles of chemicals stored above 3 compartment sink.
Corrective Actions: All chemicals are to be stored at a designated area away from equipment and prep areas. Store chemicals at hip level or below.
7-201.11 Poisonous/toxic materials or chemicals stored properly.
45 Observed missing ceiling tiles in main lobby area. Employee explained that due to the heavy rain from the previous nights, ceiling tiles were water damaged. He also disclosed that owner is currently working on replacing the missing ceiling tiles.
Corrective Actions: Ensure that missing ceiling tiles are replaced.
6-501.11 Repairing. The physical facilities shall be maintained in good repair.
45 Observed trash and other debris below and behind equipment in back room.
Detailed cleaning is needed for back room as well as storage area.

Corrective Actions: Missing ceiling tiles are to be replaced and tiles with water damage are to be repaired.

34 Observed full gnat trap. Ensure to change out traps once full, ensure regular pest control procedures.
228.186(k)(2) Controlling pests. Check premises.
42 Observed accumulation of black residue in ice machine. Ensure to regularly clean ice machine to avoid buildup of black residue.
228.113(3) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
43 Observed buildup of orange residue on vents in back storage area. Ensure vents are cleaned regularly to avoid buildup of orange residue.
228.186(d)(1) Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials.
45 Observed missing ceiling tiles in main lobby, manager stated this is due to the air conditioner being fixed because of a leak. Ensure to replace ceiling tiles once done working on air conditioner.
