39 Observed bottled water packs on the ground next to trash can. Ensure all food items are kept 6 inches or 15 centimeters off of the ground as per 4-903.11(D) Items that are kept in closed packages may be stored less than 15 cm (6 inches) above the floor on dollies, pallets, racks, and skids that are designed as specified under §4-204.122 of this title.
37 Observed open sugar bag underneath coffee machine in cabinet. Ensure all food is stored in either a package, covered container or packaging in order to avoid cross contamination as per 3-302.11(A)(4) Food protected from cross contamination by storing the food in packages, covered containers, or wrappings.
29 Observed concentration of chlorine made that was unable to be tested by a test kit. Ensure sanitizing concentration is accurately determined by a test kit as per 4-501.116 Concentration of the sanitizing solution shall be accurately determined by using a test kit or other device.

45 Observed holes in ceilings and walls in the facility. Ensure the facility is maintained in good repair as per 6-501.11 Repairing. The physical facilities shall be maintained in good repair.
32 Observed broken racks in walk-in cooler with clear plastic tape as a repair. Ensure all equipment is design and constructed with safe materials and in good repair as per 228.223(h) Equipment and utensil design and construction. safe materials and maintained in good repair.
42 Observed cold hold container with brown residue in between the back of the racks. Ensure all nonfood-contact surfaces are kept free from any accumulation of debris, dust, dirt or food residue as per 4-601.11(C) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
