31 Observed that the men’s bathroom handwash sink only had cold water that reached 73 degrees Fahrenheit (F). The hot water handle did NOT produce hot running water. The women’s bathroom has two handwash sinks with cold water that only reaches 73 to 74 F. There is only one twist open knob at each sink. Ensure that a new faucet is installed with both hot and cold-water handles. Ensure that all kitchen and bathroom handwash sinks have cold and hot running water. The hot water MUST reach a MINIMUM of 100 degrees Fahrenheit (F) during all hours of operation. The hot water for the handwash sink CANNOT be lower than 100 F. Ensure the following: 5-202.12(A) Hand washing facility provide water at a temperature of at least 38 degrees Celsius (100 degrees Fahrenheit) through a mixing valve or combination faucet.

31 Observed that the two handwash sinks in the small kitchen area did NOT have hand soap or disposable paper towels. Ensure that all kitchen and bathroom handwash sinks are stocked with hand soap and disposable paper towels during all hours of operation. Ensure the following: 6-301.11 Hand washing cleanser, availability. 6-301.12 Hand drying provision.
31 Observed that the handwash sinks in the women’s and unisex bathrooms that are used by the students did NOT have hot running water. Observed that the unisex bathroom had a hot water handle that did NOT let the hot water run. The temperature at all the bathroom handwash sinks were 79 degrees Fahrenheit (F), when it NEEDS to reach 100 F MINIMUM. The inspector has instructed the staff and students to use the handwash sinks in the small kitchen when they wash their hands (kitchen handwash sinks reached well above 100 F). Ensure that a maintenance order is placed to have the handle working again at the unisex bathroom. Ensure the following: 5-202.12(A) Hand washing facility provide water at a temperature of at least 38 degrees Celsius (100 degrees Fahrenheit) through a mixing valve or combination faucet.
37 Observed apples and oranges stored on a plate in the open. Ensure that all fruits are stored in a closed container with a lid, cover, or wrapping. This shall prevent contamination from outside or miscellaneous sources when the students or staff are not on site. If the staff wants t leave the fruit out in the open, then a sneezeguard or some form of barrier is REQUIRED. Ensure the following: 3-302.11(A)(4) Food protected from cross contamination by storing the food in packages, covered containers, or wrappings.
45 Observed a gap around the plumbing that goes into the wall underneath one of the handwash sinks in the kitchen. Ensure that the gap is caulked or sealed. Ensure that all walls, floors, and ceilings are smooth, easily cleanable, non-absorbent, durable, and in good repair. This shall reduce the number of points of entry for pests. Ensure the following: 6-501.11 Repairing. The physical facilities shall be maintained in good repair.

34 Observed a multitude of gnats near the 3-compartment sink, within the mop sink closet, and outside of the walk-in cooler. Pests must be eliminated w/ a licensed pest control company. Ensure the following: 6-501.111(D) Controlling pests. Eliminating harborage conditions.
19 Observed leak underneath the hand sink nearest the serving line. Observed leaks in the faucet/ valves of the 3-compartment sink. Observed leak underneath the hand sink nearest the exit in the back. All kitchen sinks must be in good repair with no leaks. Ensure the following: 5-205.15(A) A plumbing system shall be repaired according to the Plumbing Code.
23 Observed that the hot water of the mop sink only reached 102 F. Hand sinks should reach 100 degrees Fahrenheit, and the 3-compartment sinks and mop sinks should reach 110 F. Ensure the following: 5-103.11(B) Does the establishment have sufficient capacity to meet peak hot water demands?.
