24 Observed bags of ice being sold without proper labels. The bags of ice, if the ice is made in the ice machine within the establishment, NEED to have a label with the following printed on it: 1. Common Name of the food item or beverage. 2. Name of the business/ establishment 3. Address of the kitchen where the food item or beverage was prepared/ cooked. Failure to produce these labels and place them on the nags of ice shall disbar the establishment from selling bags of ice to consumers. Ensure the following: 3-602.11(A) Food packaged in a establishment, shall be labeled as specified in law. 3-602.11(B)(1) Label include the common name of the food, an adequately descriptive identity statement. 3-602.11(B)(4) Label include the name and place of business of the manufacturer, packer, or distributor.
31 Observed that the bathroom handwash sink was only reaching 79 degrees Fahrenheit (F). The handwash sink is REQUIRED to reach a MINIMUM of 100 F during all hours of operation. Ensure the following: 5-202.12(A) Hand washing facility provide water at a temperature of at least 38 degrees Celsius (100 degrees Fahrenheit) through a mixing valve or combination faucet.
33 Observed that both the three-compartment sink, and the mop/ service sink were only reaching 79 degrees Fahrenheit (F) in temperature. Ensure that the three-compartment sink, and the mop/service sink reach a MINIMUM of 110 F during all hours of operation. Failure to return the hot water to the establishment shall result in the business being disbarred from selling, pickles, eggs, milk, coffee, and flavored slurries.
18 Observed Odorless Lighter Fluid stored alongside Ready to Eat (RTE) food items such as Doritos, and stored above sodas. Ensure that chemicals and pesticides are stored below or separated from food items, beverages, single-use items, and food-contact utensils. Ensure the following: 7-201.11 Poisonous/toxic materials or chemicals stored properly.
27 Observed a Coca Cola cooler with high internal temperatures of 76 degrees Fahrenheit (F). Four cartons of eggs were stored inside. The inspector instructed the staff to move the eggs to the walk-in beer cooler to keep the eggs at or below 41 F. All cold holds, reach-in coolers, and walk-in coolers MUST always be in good repair to keep the cold Time/ Temperature Control for Safety (TCS) food at or below the REQUIRED 41 F. Ensure the following: 4-301.11 Cooling, heating, and holding capacities. Equipment.

45 Observed ceiling tile with a hole and water damage above non TCS packaged food items. Going into the bathroom, at the top left corner, it must be cleaned and sealed/caulked. Large hole to the right of the water heater must be sealed/ caulked. Observed missing tiles/ uneven floor within the walk-in cooler. Floor must be smooth, durable, non-absorbent, in good repair, and easily cleanable. Ensure the following: 6-501.11 Repairing. The physical facilities shall be maintained in good repair.
46 Observed a leak in the plumbing connected to the air vents within the walk-in cooler where the soft drinks and beer are stored. Leak is dripping into a large trash can filled with water. Ensure the trash can is being emptied out frequently. Observed the mop sink without a hot water knob to turn on the hot water. Ensure the following: 5-205.15(B) A plumbing system shall be maintained in good repair.
23 Observed no hot water at the bathroom hand sink, the three compartment sink, and the mop sink. The establishment should always have hot water as per: 5-103.11(B) Does the establishment have sufficient capacity to meet peak hot water demands?
39 Observed ice scooper laying on an expired sealed antifreeze container. The ice scoop needs to be sanitized and placed in its own container/ holder with the handle outside for the employee to grab. Observed tongs to grab pickles on a single use item (paper plate). The tongs need to be stored in a bucket/ container with the hand contact portion sticking out for customers to easily grab. Ensure the following: 3-304.12(A) In-use utensils, between-use storage. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored except as specified under subsection (B) of this section, in the food with their handles above the top of the food and the container.
36 Observed cloth near coffee machine, dry and not placed within sanitization bucket. Ensure the following: 3-304.14(B)(1) Cloths in-use for wiping between uses stored.
