1 Observed refried beans in 2 different containers, both made yesterday, to be sitting at 49 and 50 degrees F. One container of beans was in walk in cooler and the second container was in reach in cooler in back kitchen. Ensure to cool in the following manner prior to storing in refrigeration units: Once food is removed from cooking and reaches 135 degrees F, food is allotted 2 hours max to reduce to 70 degrees F. Once food reaches 70 degrees F, food is then alloted 4 additional hours, if needed, to reduce from 70 to 41 degrees F. Food can be covered and stored properly once it reaches 41 degrees F.
18 Fly trap bag was hanging above preparation table where fruit is cup. Ensure that all pesticides and such items are stored in areas away from food and food contact surfaces.
27 Cold hold unit in front of grill area was not holding proper temperature. Thermometer in unit, read 70 degrees F at time of inspection. Much water pooling along bottom of unit where foods are being stored. Ensure unit is serviced to ensure holding cold temperatures of 41 degrees F and below.
37 Ensure all foods in walk in cooler are properly covered at all times. Containers of food should not be stored on pooling water along bottom of cold hold unit.

1 Improper cooling procedures are being conducted. Ensure to cool in the following manner: 1. Once food is removed from cooking and reaches 135 degrees F, food is alloted 2 hours maximum to cool from 135 to 70 degrees F. Once food is at 70 degrees, food is alloted 4 hour maximum to reduce from 70 to 41 degrees. Food can then be covered and stored.
10 Observed ice machine to have buildup. Observed pooling water at cold hold unit. Ensure to clean and remove pooling water and buildup. Ensure that all foods are stored on clean and dry surfaces to prevent contamination.
32 Broken sliding window in ice machine should be repaired. Back door leading to outside from storage room should be sealed properly. Handle on lid from cold hold unit should be replaced. All surfaces should be non absorbent, easily cleanable, and non absorbent.

1 Refried beans prepared day prior were reading at 49 degrees F. Ensure that cooling is done properly and within proper time restraints as follows, to ensure storage is maintained at 41 degrees F and below: Once food is removed from cooking step and reaches 135 degrees F food has 2 hours to cool from 135 to 70 degrees F. Food then has 4 hours to col from 70 to 41 degrees F, if needed. Use ice baths, ice sticks, stirring, monitoring, etc to ensure done properly.
10 Ice machine had black residue along interior that should be thoroughly cleaned and properly sanitized to remove residue and maintain machine clean. Ensure that sanitizing water for cloth towels is only made of bleach and water. No soap should be added to solution. Thoroughly clean rubber gaskets in refrigeration units to remove buildup of food.
