47 Observed expired permit posted. Employee contacted manager and then the manager found the permit and sent a photo. Ensure permit is posted as per 8-304.11(A) Permit posted.
46 Observed the ladies restroom door unable to self-close. Manager was aware of the broken door and contacted was going to fix it today. Ensure restroom door self-closes to prevent recontamination of hands as per 6-501.19 Closing toilet room doors.
45 Observed cracks and holes on the floor in the bar area as well as by the ladies restroom. Ensure floors are kept smooth, durable, and easily cleanable as per 6-101.11(A)(1) Materials for floor, wall, and ceiling shall be smooth, durable, and easily cleanable.
45 Observed missing ceiling tiles in the bar area and women's restroom. Ensure the facility is kept in good repair as per 6-501.11 Repairing. The physical facilities shall be maintained in good repair.
37 Observed jugs of juice on the floor in the walk-in cooler. Ensure all food is at least 6 inches or 15 cm off of the ground as per 3-305.11(A)(3) Storing the food at least 15 cm (6 inches) above the floor.
39 Observed ice machine with accumulation of soil or black/white residue inside the inner edges. Ensure ice machine is frequently cleaned to prevent an accumulation of soil or mold as per 4-602.11(E)(4) Surfaces of utensils and equipment contacting food that is not time/temperature control for safety shall be cleaned: in equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (a) at a frequency specified by the manufacturer; or (b) absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.

During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in a clean, protected location if the utensils, such as ice scoops, are used only with a food that is not time/temperature controlled for safety.

NOTE: The City of San Antonio Metropolitan Health District presents food establishment "Inspection Scores" calculated from a perfect 100 score minus any identified violations and subsequent demerits.
