1 Observed cooling of refried beans beginning. Employee explained an improper cooling procedure. No thermometers were presented which should be used for cooling to be conducted in the following manner: Once food is removed from heat and cools to 135 degrees F, food is allotted 2 hours to cool from 135 to 70 degrees F. Food is then allotted 4 additional hours to cool from 70 to 41 degrees F. Food should not be stored until at cold holding temperatures of 41 degrees F and below. Chicharron guisado prepared yesterday was at 52 degrees F. Carne guisada prepared yesterday was at 56 degrees F. These items will be dscarded and unable to be used for customers. Provide thin probe thermometers to cool properly.
2 Shredded cheese container was stored on pooling water and at 70 degrees F. Cheese was being stored in same area of hot holding unit. Observed carne asada, in cold hold unit, to be at 51 degrees F. Ensure that all foods held at cold hold units, are held at 41 degrees F and below at all times.
10 Cloth towels were being stored in solution without any bleach. This container was being stored in hand sink. Observed dishes being washed improperly. Dishes were being washed with soap and bleach solution and then rinsed with bleach water. Ensure that wares are washed properly as follows: 1. WASH (soap and water) 2. RINSE (water) 3. SANITIZE(bleach water solution at 50-100ppm concentration) Observed ice machine to be dirty along interior. Ensure that surfaces are cleaned thoroughly to remove buildup.
18 Tortilla chips were being stored in plastic to go grocery bags. Observed rice and flour being stored in large plastic sterilite containers. Ensure that all foods are stored in food grade materials only. Bleach concentration was high for cloth towels solution at waitress area. Ensure to maintain concentration between 50-100ppm.
27 Reach in cooler near back drive thru area was reading at 50 degrees F, as well as cold hold unit nearest ware washing which was at mid 50s. Ensure that these units are serviced and ensure that cold hold temperatures are able to be maintained at 41 degrees F and below. Foods must be able to be held at proper cold temperatures at all times.
29 Provide thin probe thermometers for employee use. Provide thermometers in all refrigeration and cold hold units. Provide test strips for bleach water.
31 Hand sink did not have hand soap available. Hand sink had container with cloth towels in sanitizing solution being stored in hand sink. Ensure that hand sink is readily available and only used for hand washing practices.
37 Ensure all foods stored in coolers are properly covered. Observed plastic bins with masa balls for tortillas stored on the floor. Issue was corrected. Observed large bins with ice being stored on floor when filled with ice then placed on preparation table for cooling. Ensure that all foods and wares are stored 6 inches or more off floor.
45 Observed personal beverages being stored on food and food contact surfaces. Ensure that all personal drinks are stored in a designated area away from food and food contact and that they are stored in a cup with lid and straw only.

1 Observed refried beans that were prepared yesterday sitting at 48 degrees F. Several other food items were being cooled at this time. Foods should be cooled in the following manner: Once removed from heat and reaches 135 degrees F, food should be cooled to 70 degrees F within 2 hours. Food should then be cooled from 70-41 degrees F, within 4 hours. Foods should be stored at 41 degrees F and below.
18 Bleach water solution is being used for sanitizing of cloth towels, used for wiping surfaces. Concentration was too high and should be maintained between 50-100ppm. Grocery style to go bags should not be used for food storage in hot holding table or for tortilla chips, etc. Only food grade bags and materials should be used for food storage. Lysol spray can should not be stored next to beverages. All toxics should be stored in a designated area away from food and food contact surfaces.

1 Refried beans that were prepared yesterday were reading at 61 degrees F at time of inspection. Proper cooling procedures are not being followed. Ensure to cool in the following manner: Once food is removed from heat and is at 135 degrees F, food is allowed 2 hours max to cool from 135 to 70 degrees F. Food is then allowed 4 hours max, if needed, to reduce temperature from 70 to 41 degrees F. Once food has reached 41 degrees F, food can be covered and stored. Container of refried beans were discarded by manager as instructed by inspector.
15 Observed employees handling and preparing ready to eat foods (tacos, tortillas, shredded cheese etc) with bare hands. Manager stated they would rather use gloves as a barrier at this time, to correct issue. Issue was corrected on site. Ensure to always use a barrier (i.e. deli tissue, utensils, gloves, etc) when handling ready to eat foods.
18 Observed to go style bags and can liner bags being used to store corn tortillas and masa in. Ensure that all foods are stored in only food grade storage material.
20 Leakage observed from hand sink after washing hands. Ensure that leaks in drain pipes are corrected and properly disposed into proper sanitary drains.
27 Ensure to use some of the following methods for proper cooling, as was observed to be a violation in refried beans. -follow proper time and temperature control -continuous stirring of foods -ice sticks -ice baths -foods in smaller portions -etc.

31 At beginning of inspection, hand sink was blocked by trash can, and had container being stored in sink. Ensure to have hand sink readily available at all times.
