10 Sanitizing solution for cloth towels, was being stored in improper concentration. Ensure that when using bleach water for sanitizing, that concentration is maintained between 50-100ppm.
22 Ensure all employees who handle, prepare, wash, etc foods and wares, and are not Food Manager certified, should be Food Handler certified, with certificates made available upon request.
23 Hot water not available at hand sink in kitchen or restroom, and in three compartment sink. Manager will check water heater to verify if tank size is sufficient for what is done in establishment, to ensure hot water is available where needed, at all times.
25 Shrimp ceviche is prepared and served on site. Raw shrimp is 'cooked' with lime juice. Proper written procedures are not in place at location, and will need to have a write up, on procedure from raw product to the point of sale. Establishment should write up a letter requesting a variance. There is not specific format at this time, on how letter and request should be written. Ensure that the request states the item that is made, and that it does against the health code, in regards to cooking raw shellfish. Once variance, is submitted, a statement describing procedure on how item is prepared from raw to point of sale, will be requested by health department. Please ensure variance request is submitted within 10 day follow up to inspector.

31 Hand sink was blocked and had many dirty wares in sink. Ensure that hand sink is only used for hand washing purposes. Paper towels and hot water were not available at hand sink. Ensure to provide paper towels and hot water along with hand soap. Observed area where drinks are made, near restroom, to not have a hand sink at location. Ensure that if open handling is occuring in this area, that adequate and supplied hand sinks are available. Owner stated that all beverages would be prepared in kitchen. If procedures and observations are made, differently, in future, a sink may be requested to be installed in this area.
32 Long white cutting boards where tacos are prepared, had many deep cuts and scarring. Ensure that all boards, including all wares, are in sound condition to be smooth, easily cleanable, and non absorbent.
